Increase Windows MAK activation limit

In order to increase the MAK activation limit for customers that have exceeded MAK activations due to re-imaging machines etc, fill in the blanks in the email template below and send to makadd@microsoft.com:

    a. Limits are being reached due to devices being re-imaged every few weeks. KMS currently is not available within the environment.
7. Time frame: Can this please be completed ASAP due to devices not being licensed.
